Why Cracked Teeth Are So Easy to Miss

Unlike a cavity you can sometimes see, or a broken tooth that is obvious, a crack often gives no visible warning. The pain comes and goes. It appears when you bite down at a certain angle, then disappears completely. Some patients notice cold sensitivity that fades within seconds. Others feel nothing at all.

Research shows that up to 70% of adults have at least one posterior tooth with a visible crack, yet only about 20% of those teeth produce any symptoms.¹ That gap between having a crack and feeling it is precisely why so many people wait too long before seeing a dentist.

What Happens When a Crack Goes Untreated

A crack does not stay the same size. Every time you chew, bite, or clench, the two sides of the crack flex against each other. Over time, that movement pushes the crack deeper.

The concern is not only structural. Crack lines harbour bacteria in biofilms that work their way into the dentinal tubules, triggering inflammation inside the pulp, which is the living nerve tissue at the centre of the tooth.¹ What starts as mild sensitivity can progress to irreversible pulp damage, at which point root canal treatment becomes necessary.

A large clinical study of nearly 2,900 patients with cracked teeth found that biting pain, spontaneous pain, and radiographic evidence of a crack were all strong predictors of needing restorative treatment rather than simple monitoring.² The longer those signs go unaddressed, the further the crack tends to progress.

The Symptoms Worth Taking Seriously

Not every cracked tooth needs urgent treatment, but certain symptoms call for a same-week appointment:

Sharp, brief pain when biting down that disappears quickly. Cold sensitivity that lingers for more than a few seconds. Discomfort that seems to shift around the tooth without a clear source. Intermittent swelling near the gum around a single tooth.

When a crack has progressed and root canal treatment becomes necessary, survival rates after endodontic treatment remain comparable to non-cracked teeth that receive the same procedure, generally between 84% and 90%.¹ The key factor in both situations is not waiting.

Do Not Wait for the Pain to Get Worse
